Buckshaw Primary School will not give your child prescribed medication unless you have completed and signed the parental request form.  All medicine should be dropped off at the school office.

I confirm that the above information is accurate, and I give consent to the school staff administering medicine in accordance with the school medicine policy.  I will inform school immediately, in writing, if there is any change to the dosage or frequency of the medication.

I understand I must deliver the medicine personally to the school office.
